Activity Club days are 8:30am to 3:30pm and held at the Maths School, Rochester on the following dates:
£30 per day per participant
Reservations must be confirmed by payment in full prior to attendance.
In case of an emergency and for medical care needing to be given, during the participants attendance at the Activities Club, please ensure that you provide the name of your GP, Surgery Details and all medical history for the participant, including all known allergies, any SEN requirements and anything else important for H20 to be aware of.
By completing and submitting this form, you also give permission should the appropriate emergency care need to be given by trained H20 Members of Staff or additional Emergency Services Personnel. Should you need to specify any additional information in this instance, please add it to the required information below.
All information provided will be treated in the greatest confidence.
During the club days, photographs and videos are taken of children during the numerous activities. These photos/videos can be posted on social media during the period of the activities club and may be used for advertising purposes at a later date on leaflets, posters, social media, etc. Using the box below, please indicate whether you consent or refuse to photographs being taken:
